Job Application Form
Personal info, contact details, position applied for, resume upload, cover letter upload, LinkedIn URL, years of experience, references (via repeater), availability, salary expectations. The most complete job application template.
Entry-Level Job Application
Shorter version for entry-level roles: personal info, resume, short essay on why you’re interested, availability. Faster to complete, better completion rates for high-volume applicant pools.
Internship Application
Student info, school, major, graduation year, resume, transcript upload, letter of recommendation upload, availability, why you’re interested. For companies with internship programs.
Volunteer Application

Key features for application forms
File uploads
Application forms almost always need file uploads — resumes, transcripts, portfolios, certifications. Form Forge’s file upload field supports:
- Multiple files per field
- Configurable size limits (typical 10–25 MB)
- Allowed file types (PDF, DOC, DOCX common)
- Secure storage outside the public directory
Multi-step format
Long application forms fare much better as multi-step forms with a progress bar. Our application templates default to multi-step with 3–4 logical sections.
Conditional logic
Conditional logic is heavily used in applications:
- “Are you authorized to work in the country?” → if No, show visa sponsorship question
- “Years of experience?” → if 0–2, show different questions than 10+
- “Do you have references?” → if Yes, show the references repeater
Repeater fields for references and work history
Applicants rarely have exactly 3 references or exactly 2 previous jobs. The repeater field lets them add as many as they need, with a reasonable maximum.
Signature field (for consent and terms)
Drawn signature field for “I certify the above information is true” confirmations common in legal and employment applications.

Admin email with attachments
The admin receives an email with every field value and the uploaded files (as attachments or download links, depending on file size).
PDF of the application
