Quote Request Form
Client info, project description, requirements, budget range, timeline, preferred contact method. No payment — just captures the info needed to respond with a quote.
Invoice Generation Form
Client info, line items (via repeater field with description, quantity, unit price), automatic subtotal + tax + total calculations. Submit and a PDF invoice is generated.
Catering Order Form
Event type, date, guest count, menu selection, dietary requirements, delivery address, payment. For restaurants and catering businesses.
Print Order Form

Key features for order forms
Stripe payments built in
Every order template uses Form Forge’s Stripe payment field. Card data never touches your WordPress server — it’s PCI-compliant out of the box. Test/live mode toggle in settings.
Calculation fields for totals
Order templates rely heavily on calculation fields to compute subtotals, taxes, discounts, shipping, and final totals. Totals update live as the user makes selections, so there’s no “calculate” button needed.
Line items with repeater fields
For orders with variable number of items, the repeater field lets users add as many products or services as they need. Each row has its own product, quantity, and price sub-fields; the calculation field sums across all rows.
Automatic tax calculation
A standard formula like {subtotal} * 0.08 computes an 8% tax automatically. Or use conditional logic to apply different tax rates based on shipping state/country.
Discount codes
Add a text field for a discount code, and a conditional calculation to apply the discount only if the code matches a known value. For more complex discount systems, use a webhook to validate codes server-side.
Order confirmation emails
Order templates send:
- To the customer — a receipt with line items, total, and a thank-you message
- To the admin — a full order notification with all fields
Both are customizable. Optional: attach a PDF of the order as a receipt.
HubSpot or CRM integration
Push order data into HubSpot CRM as a deal with the appropriate pipeline stage, or use webhook integration to send to any other system.
