Form Forge’s 6 donation form templates are built for nonprofits, charities, and fundraising campaigns. Stripe payments handle the money directly to your organization’s account (Form Forge takes no cut), preset amounts reduce decision friction, and optional dedication fields let donors give in honor of someone.

Tithe or offering selection, member vs guest, preset amounts or custom, payment frequency, payment.
Every donation template uses Form Forge’s Stripe payment field. The donation goes directly to your organization’s Stripe account — Form Forge takes no fee beyond the WordPress plugin license. Test/live mode toggle.
Research shows preset amounts convert better than open-ended “enter an amount” fields. Donation templates default to 4–5 presets with a “Custom amount” option. Amounts are configurable per form.
Optional “in honor of” or “in memory of” fields with conditional logic — they only appear if the donor checks “This is a dedication” at the start.
For subscription-based giving, the Stripe payment field supports recurring charges configured as weekly, monthly, quarterly, or yearly. Donors manage their subscriptions via a self-service portal linked in the receipt email.
Donation templates automatically generate a tax-deductible receipt (PDF) that includes your organization’s tax ID, the donation amount, date, and donor info. Sent to the donor as an email attachment or available as a download link.
Push donor data into HubSpot CRM, Mailchimp, or Salesforce via the built-in integrations — add donors to a supporter list, track campaign attribution, trigger thank-you workflows.
Optional “Make this donation anonymous” checkbox that hides the donor’s name from any public display but keeps it in your records.
For campaign-specific donation forms, Form Forge can display a progress bar showing the current total toward your campaign goal. This builds urgency and social proof — donors who see a campaign close to its goal are more likely to contribute.
The progress bar updates automatically as donations come in.
Dedicated donation plugins (GiveWP, Charitable, etc.) offer more fundraising-specific features: fee coverage options, peer-to-peer fundraising, donor profiles, reporting. Form Forge’s donation templates are for:
