User Registration (Website Account)
Standard user signup: username, email, password, confirm password, terms checkbox. Integrates with Form Forge’s User Registration feature to create a real WordPress user account on submission.
Event Registration
Name, email, phone, number of attendees, dietary restrictions, session preferences. Great for conferences, workshops, and webinars.
Conference Registration with Payment
Full conference signup: personal info, ticket type (radio with prices), optional add-ons, Stripe payment field for the total. AI calculates the total based on selections.
Membership Signup
Member name, contact info, membership tier selection, payment (optional), terms acceptance. For clubs, associations, and paid memberships.
Course Enrollment
Student info, course selection (from a dynamic list), payment plan options, emergency contact. Works for online courses, workshops, or educational programs.
Newsletter + Account Combo

Common features across registration templates
User account creation
Registration templates integrated with Form Forge’s User Registration feature automatically create a WordPress user account on successful submission. Configure the role (subscriber, contributor, customer, etc.) per form.
Email list integration
One checkbox enables Mailchimp subscription on the registration. The user is added to your audience with tags based on form fields (membership tier, interest, location).
Payment support
Any registration template can be extended with a Stripe payment field for paid signups. Calculation fields auto-compute totals based on membership tier or ticket type.
Confirmation emails
Registration templates send two emails by default:
- To the admin — with full submission details
- To the registrant — confirming registration and providing next steps
Both are customizable.
Double opt-in
For newsletter or email list registrations, enable double opt-in: the registrant receives a confirmation email with a verification link before being added to the list. Reduces spam and keeps your sender reputation clean.
Data protection
Registration forms collect personal data by definition. Every Form Forge registration template includes:
