Using a template does not mean accepting someone else’s design. It means getting a head start. Every template is fully editable the moment you apply it — you can change labels, add fields, remove fields, reorder everything, and adjust validation. Think of templates as professional blueprints that you customize to fit your exact requirements.
How to Browse and Use a Template
- Go to Form Forge > New Form.
- Click Use Template at the top of the builder. The template gallery opens.
- On the left side, browse the 27 categories. Click a category to filter templates.
- Alternatively, type a keyword into the search bar. Try terms like “contact,” “registration,” “feedback,” “order,” “application,” or “survey.”
- Click on any template to preview it. The preview shows every field with its label, type, and configuration.
- When you find a good match, click Use This Template. The form is created instantly in the builder.
- Customize anything: rename fields, change dropdown options, add or remove fields, adjust validation rules.
- Give the form a title that makes sense for your site (for example, “Contact Us” instead of the template’s generic title).
- Click Save Form.
- Embed the form on a page using a shortcode or Gutenberg block.

On the free plan, you have access to a selection of basic templates covering the most common form types (contact, feedback, simple registration). PRO unlocks the full library of 168 templates, including specialized templates for healthcare, legal, insurance, real estate, and other niche categories.
> Tip: Even if a template does not match your exact needs, it can save you time by giving you a starting structure. Pick the closest match, then modify it. Starting from a template with 10 fields and removing 3 is faster than building 7 fields from scratch.
> Good to know: Templates are a one-time starting point. Once you apply a template, the form is entirely yours. Changes to templates in future plugin updates will not affect forms you have already created. You can also use the same template multiple times to create different forms.
Common Mistakes to Avoid
- Spending too long searching for the “perfect” template. Find one that is close enough and customize it. Editing is fast.
- Forgetting to customize the template’s default labels and placeholder text. Generic text like “Enter your name” is fine, but personalized text like “Your full name” feels more polished.
- Not reviewing the template’s validation rules. Some templates have strict validation that may not fit your needs. Check each field’s Required setting and adjust.
